Disclosure in Family Proceedings

The London Request Form (LRF) was introduced on 05 August 2013, to replace the national Police/ Family Disclosure Protocol within the London area. The LRF provides timely advance notice to police of the existence of family law proceedings, and the scope of police information sought. The LRF encourages police and parties involved, including children social care, to share information at the earliest opportunity when family cases are contemplated.

The LRF has the support of The Judiciary and representatives from the local authority.

Making a request for disclosure in Family Proceedings

The relevant LRF is available for download via the Related publications section on this page.

Details of the MPS General Fees and Charges, and guidance on how to complete the form is available for download via the 'Related links' and 'Related publications'.

The Met needs to be satisfied that disclosure is being sent to the individual detailed on the form and consequently unrepresented parties will be required to submit an original signed application form and documentation to prove identity. Unfortunately in these cases we cannot accept forms sent via email.

Please note: Incomplete requests will be rejected.
